With the following information:

• Your name (first, last)
• Name of Person/Animal
• Birth & Death month/day/year
  • Please spell out the month (abbreviation), with a 4-year year
  • EX: 31 Jan 2005
  • dates such as: 1/10/05 can be :
    • Jan 10 2005
    • Oct 1 2005
    • Oct 5 2001
    • and others
• Pictures (as attachments)
• Memorial text (either as HTML, Word, text format)
